Sculptra is an injectable that may help your skin rebuild collagen in areas where the face has lost volume, such as hollow cheeks and deep lines around the nose and mouth. Results come on slowly. Most people need about three sessions spaced several weeks apart, and the change shows up over the following months. We offer Sculptra at our Spring Branch and New Braunfels clinics after a consult with one of our nurse practitioners.
In clinical studies, results lasted up to about two years. Because the change is gradual, Sculptra suits people who want to look less tired over time without a sudden change.
What is Sculptra?
Sculptra is a prescription injectable made by Galderma. Its active ingredient is poly-L-lactic acid, or PLLA, a material the body breaks down over time. Before each treatment, the powder in the vial is mixed with sterile water to form a liquid that can be injected.
The FDA has approved Sculptra for shallow to deep smile lines (nasolabial folds) and other facial wrinkles in adults with a healthy immune system. In 2023, the FDA added fine lines and wrinkles in the cheek area to that approval.
How does Sculptra work?
Collagen is the protein that keeps skin firm. Your body makes less of it as you age, and sun exposure speeds up the loss. Sculptra places tiny PLLA particles in the deep layer of the skin where collagen is made. As the particles break down, your body builds new collagen around them.
That is why Sculptra is called a biostimulant. Hyaluronic acid filler adds volume the day it goes in. Sculptra gets your own tissue to add it gradually.
How many Sculptra sessions will I need?
The FDA label describes a series of one to four sessions, with three being typical. Your provider checks your progress about three to four weeks after each visit and decides whether you need another. How many you need depends on how much volume you have lost and how many areas we treat.
We set the number of sessions and the spacing between them at your consult, and we adjust the plan as your results come in.
Why it can look finished on day one
Right after a session, the area can look fuller. That is swelling plus the water used to mix the product. It fades within a few hours to a few days, and you may look the way you did before treatment. This is expected. The collagen change takes weeks to show, which is why providers inject a little less than full correction at each visit.
How long do Sculptra results last?
In a U.S. clinical study, results for some patients lasted up to 25 months after the last session. In the study that led to the cheek approval, most patients still had results at 24 months. How long your results last depends on how much volume loss you started with and how your skin keeps aging.
Which areas can Sculptra treat?
Sculptra's FDA approval covers smile lines, other facial wrinkles, and fine lines and wrinkles in the cheek area. At our clinics, we use it for hollow cheeks, smile lines, and marionette lines, which run down from the corners of the mouth.
If you want to improve another area, ask at your consult and we will tell you what fits.
Who is a good candidate for Sculptra?
Sculptra suits adults who have lost volume gradually and want a slow, natural-looking change rather than an instant one. Common reasons people come in are cheeks that look flatter than they used to, a face that looks tired or thin, and lines around the mouth that have deepened.
It is a good match if you are patient with results. If you have an event next weekend, a hyaluronic acid filler is usually the better tool.
Who should wait or skip it
The label says not to use Sculptra if you have had a severe allergic reaction such as anaphylaxis, have several severe allergies, are allergic to any ingredient in it, or tend to form raised scars or keloids. If lidocaine is mixed in, a lidocaine allergy also rules it out.
Tell us if you take medicines that suppress the immune system, take blood thinners such as aspirin or warfarin, or have a bleeding disorder. Mention any recent laser treatment, peel, or hair removal in the area. Sculptra has not been studied in people under 18 or during pregnancy or breastfeeding. If you have acne, a rash, a cold sore, or an infection where we would inject, we wait until it heals.
What happens at a Sculptra visit?
Your first visit starts with a consult. A nurse practitioner looks at your facial structure, skin, and goals, goes over your health history and medications, and tells you whether Sculptra makes sense.
On treatment day, you remove your makeup and we clean the skin. We may apply a numbing cream first. The provider then places many small injections with a fine needle and massages the area to spread the product evenly. A session usually takes 30 to 60 minutes. Most people go back to their normal day right after.
Aftercare and massage
The label tells patients to massage the treated area for five minutes, five times a day, for five days. This helps spread the product evenly. Your provider will go over how to do it before you leave.
During the first 24 hours, use an ice pack wrapped in a cloth to reduce swelling. For that same day, avoid hard exercise, long sun exposure, tanning beds, and extreme heat or cold. You can usually put makeup back on a few hours after treatment if the skin is not bleeding or open.
Side effects and risks
In clinical studies, the most common side effects were swelling, redness, tenderness, bruising, itching, and small bumps at the injection sites. Most were mild to moderate and went away within one to two weeks. About 71 percent of patients felt some pain after their first session, 14 percent rated it moderate, and most of it was gone within a day.
Small lumps under the skin, called papules or nodules, can appear weeks or months after treatment. Some go away on their own, and others need treatment. The label notes the risk may be higher when a large amount goes into one spot, especially the deep lines at the corners of the mouth. Sculptra goes in as many small amounts rather than one large one, and the massage routine helps spread it evenly. Call us if you notice a lump that does not settle.
As with any facial injection, there is a rare but serious risk of product entering a blood vessel. Get emergency care right away if you notice vision changes, skin that turns white near the injection site, unusual pain, or signs of a stroke such as trouble speaking or weakness in your face, arm, or leg.
Sculptra vs. hyaluronic acid filler
Hyaluronic acid fillers, such as the products we use for lips and cheeks, give you a visible result the same day. They are the better choice for quick, precise changes like lip shape or a defined jawline. Our lip filler and dermal filler guide covers that side.
Sculptra works over months and tends to treat broader areas of volume loss. Some patients use both, with Sculptra to rebuild the base and filler to fine-tune specific spots. Our post on fillers vs. biostimulants compares the two approaches in more detail.
What does Sculptra cost?
The cost depends on how much product you need, which areas we treat, and how many sessions are in your plan. We give you a personal estimate at your consult before any treatment starts.
If you want to spread out the cost of a series, we offer payment plans through Affirm and Cherry. You can read about both on our pricing page.
Common questions about Sculptra
Is Sculptra a filler?
The FDA classifies it as a dermal filler, but it works differently from hyaluronic acid filler. Instead of filling the area directly, it prompts your skin to build collagen over time.
When will I see results from Sculptra?
The early fullness fades within a few days. The collagen change shows up gradually over several weeks to a few months, and it keeps building across your series of sessions.
Does Sculptra hurt?
Most people feel some discomfort from the injections. We can use numbing to help, and in studies most post-treatment pain was gone within 24 hours.
Can Sculptra go in the lips or under the eyes?
Not the lips. The label says Sculptra should not be injected into the red part of the lip, and its use around the eyes has not been studied. For lips, a hyaluronic acid filler is the usual choice.
Can I work out after Sculptra?
Skip hard exercise for the first 24 hours. Most people go back to the rest of their normal routine the same day.
Can I combine Sculptra with Botox or filler?
Often, yes. Botox relaxes the muscles that cause frown and forehead lines, filler adds instant volume, and Sculptra rebuilds collagen. Your provider will tell you which ones fit your goals and how to time them.
